You might find this thread helpful (deals with single-volume DWI data, but I bet same principles apply).

Question Regarding Validation of DWI data

I’m able to reproduce this with a 3D dwi file. You could fix this by setting dim0 to 4 instead of 3. We can open an issue on the validator to permit 3D DWI files, assuming that does not contradict the spec.
